Dr Musaddiq Nazeeri Obituary, Death; – Federal authorities are investigating a crash that resulted in the death of a prominent physician at Lancaster Airport on Sunday morning.

Dr. Musaddiq Nazeeri, aged 58, crashed a single-engine Piper PA-46 into a cornfield shortly after taking off at approximately 8 a.m.

“I heard an engine noise. I looked up from my (news) paper, and then there was just a cloud of black smoke,” recounted David Welden, a resident living adjacent to the airport. “I immediately went inside and told my wife, Jan, ‘Call 911.'”

Welden and several others hurried to the crash site to offer assistance. However, it was too late. Nazeeri was the sole occupant of the aircraft. He was declared dead at the scene. NTSB investigations involve three primary areas: the pilot, the aircraft and the operating environment.

Investigators will gather the following information and records:

  • Flight track data
  • Recordings of any air traffic control communications
  • Aircraft maintenance records
  • Weather forecasts and actual weather and lighting conditions around the time of the accident
  • Pilot’s license, ratings and recency of flight experience
  • 72-hour background check of the pilot to determine if there were any issues that could have affected the pilot’s ability to safely operate the flight
  • Witness statements
  • Electronic devices that could contain information relevant to the investigation
  • Any available surveillance video, including from doorbell cameras

A preliminary report is expected within 30 days of the accident.

Witnesses to the accident or those who have surveillance video or other information that could be relevant to the investigation are asked to contact the NTSB at witness@ntsb.gov.
